Include your full name, mailing address, telephone number, and email address where you can be contacted.
4. A statement of good faith:
Include a statement that you have a good faith belief that the use of the material in question is not authorized by the copyright owner, their agent, or the law.
5. A statement of accuracy:
Provide a statement that the information in your notice is accurate and, under penalty of perjury, that you are the copyright owner or authorized to act on behalf of the owner.

Filing a DMCA Counter-Notice
If you believe the material that was removed or disabled was wrongly removed due to mistake or misidentification, you can submit a counter-notice to our designated agent. Your counter-notice must include:
1. Identification of the material removed:
Describe the content that was removed or disabled, including the location of the material before it was removed.
2. Your contact information:
Provide your full name, mailing address, telephone number, and email address.
3. A statement of good faith:

You must also state that you will accept service of process from the party that submitted the original DMCA notice.
5. Signature:
A physical or electronic signature is required.
Upon receiving a valid counter-notice, we may restore the removed material, unless the original complainant files a court action seeking to prevent restoration.
Penalties for False Claims
Please note that anyone who knowingly misrepresents material in a DMCA notice or counter-notice may be liable for damages, including legal fees, under the DMCA. We encourage you to ensure the validity of your claims before filing a notice or counter-notice.
